Mary Utter of Artistic Expressions is responsible for collecting, storing and destroying all client personal information.  She complies and is bound by The Privacy Act and the NPPs as a small business owner.

Ms. Utter collects and uses client personal information that is relevant to and for the purpose of providing therapeutic treatment as a health service provider.  Personal information is collected from the client directly, by a guardian of the client or a professional person referring the client.

Client’s personal information is disclosed to other organisation only if Ms. Utter thinks an unlawful activity has occurred or to protect the health and safety of any person. 
 

• All client files are properly labelled and stored in a locked cabinet or protected by password on a computer.
• All personal information is destroyed 5 years after the completion of treatment.
• If personal information is not provided as requested, Ms. Utter may not be able to provide an effective therapeutic treatment.
• Clients can access their personal information by contacting Ms. Utter.
